    Patient advocacy isn’t just emotional work—it’s strategic work. The most effective case management programs don’t act in silos. They build coalitions with providers, specialists, AND other vendor partners to facilitate communication across multiple providers/specialties. They share data and risk alerts in REAL-time, as they unfold. They communicate and share evidence-based guidelines and alternatives (e.g., preemptive transplant vs default dialysis) Member support with lab monitoring, and member coaching They also hold themselves and other partners accountable to outcome metrics, not just activity volume. Anybody can bill a PEPM for absolutely no work or improvement for the members they are billing for.

My first step is always to listen—understanding patient's concerns & cultural context. It helps to communicate needs clearly & compassionately. Also working closely with medics not to challenge them, but to collaborate—as we all want best for patient. Simplifying medical info into laymen terms make patients feel empowered to make informed choices. For me, patient advocacy isn’t just a responsibility—it’s a bridge to a more human, effective, and compassionate healthcare system.

    Former pharmacy dispenser here. Honestly, I think a major issue for patients is the lack of information sharing between healthcare providers, especially hospitals, GPs, Specialists, and even pharmacy. One thing I've learnt is that a little communication goes a long way.
