    Having worked extensively in high voltage electronics, the person in question would be immediately removed as safely as possible from the given situation. A frank discussion would be had as a team to correct the issue. We would discuss what protocols were violated and determine if any other unnecessary risks were taken. We would determine what allowed the protocols to be broken and if the issues were systemic, lack of training or a pattern of behavior. Systemic and training issues can be rectified by better safeguards, changes to protocols and or remedial or updated training. Behavioral issues that show a pattern of abuse would have your keys to equipment and certification to work around HV pulled and management would take over from there.
